Iliotibial band syndrome is irritation of the connective tissue along the outer thigh and knee, causing lateral knee pain that worsens with repeated bending or straightening.

Iliotibial (IT) band syndrome develops when the thick band of connective tissue on the outside of the thigh becomes tight, inflamed, or irritated, causing pain on the outer knee or hip. The IT band runs from the hip to the knee and is essential for hip and knee stability during walking, running, and other activities. IT band syndrome typically develops from repetitive stress, muscle imbalance, poor movement patterns, or tight hip muscles. The condition is common in runners and endurance athletes but can affect anyone engaging in repetitive knee bending or hip movement. Pain from IT band syndrome is typically sharp and localized to the outer knee or hip.

Individuals with IT band syndrome commonly experience sharp pain on the outside of the knee, particularly with running or repetitive bending activities. Pain may worsen when the knee is bent to approximately 45 degrees during weight-bearing. Some people also experience pain on the outer hip. Symptoms often improve with rest but return when activity resumes. The condition often responds well to conservative care, including stretching and strengthening exercises targeting the hip and thigh, activity modification, and addressing underlying movement pattern problems.
