Rib dysfunction occurs when one or more ribs become irritated or restricted at their point of attachment to the spine.
Rib dysfunction—also referred to as costovertebral joint dysfunction—occurs when one or more ribs become irritated or restricted at their point of attachment to the spine. This can develop following an accident or injury, repetitive strain, or prolonged poor posture. Symptoms may include pain close to the spine or slightly to one side, pain that travels along the rib, or a sharp, sudden sensation often described as feeling like being “shot by an arrow.”
Individuals should notify their care provider if rib pain is accompanied by unusual coughing, indigestion, nausea, vomiting, fever, flu-like symptoms, or the appearance of a rash along the rib, as these may indicate a different underlying condition. Immediate medical attention is required if chest pain is associated with pressure, pain radiating into the arm or jaw, or shortness of breath, as these may be signs of a heart-related issue.
Most cases of rib dysfunction respond well to conservative, non-invasive care focused on restoring joint movement and reducing irritation. Temporarily limiting activities that increase pain—particularly reaching, pushing, or pulling—can support recovery. Some individuals find relief with supportive garments, topical pain-relief products, or ice applied to the affected area for 15–20 minutes.

What Is Rib (Costovertebral) Dysfunction?

This restriction may occur suddenly after strain or injury, or gradually due to posture-related stress or repetitive movement.
Common Symptoms
Symptoms of rib dysfunction may include:
- Pain near the spine or between the shoulder blades
- Pain that wraps around the chest wall along a rib
- Sharp, stabbing, or sudden pain with movement or breathing
- Discomfort when twisting, reaching, or taking deep breaths
- Tenderness over a specific rib
Pain is often reproducible with movement or pressure.
When to Seek Medical Attention
Because rib and chest pain can have many causes, immediate medical evaluation is required if you experience:
- Chest pressure or heaviness
- Pain spreading into the arm, jaw, or neck
- Shortness of breath
You should also notify your care provider if rib pain is accompanied by:
- Fever or flu-like symptoms
- Persistent cough
- Nausea or vomiting
- A rash developing along the rib (which may suggest shingles)

How Rib Dysfunction Is Typically Managed
Most cases improve with conservative, non-invasive care focused on restoring movement and reducing irritation.
Management may include:
- Temporarily avoiding activities that worsen pain
- Limiting reaching, pushing, and pulling early on
- Supportive garments to reduce rib stress
- Applying ice to the affected area for 15–20 minutes
- Using topical pain-relief products as needed
Gentle movement within comfort limits is usually encouraged as symptoms settle.
